As shown in the above graph from the Centers for Disease Control, the national rate of diabetes has increased tremendously over the past 15 years.

The Diabetes Management Task Force, comprised of representatives from the three hospitals and other area health organizations, assessed the needs of Sussex County residents and researched resources and tools for achieving better management of diabetes. The first effort will be the Diabetes Care Pilot Program, beginning with 10 Primary Care Practices (including La Red) in Sussex County from July 2012 – June 30, 2012. Through the pilot program, the task force aims to achieve the following goals:

  • Increase the percentage of Diabetes Patients in Sussex County who have completed HgbA1C testing in the last year (Medicare beneficiaries with a goal of 92%; and Medicaid beneficiaries with a goal of 75%)
  • Increase attendance and successful completion of existing diabetes education and self-management programs

Achievement of these goals will demonstrate a measurable improvement in the quality of diabetes care and self-management education that will translate into better care outcomes for diabetes patients and physician practices throughout all of Sussex County.

County-specific data from 2007-2010 demonstrate an alarming and growing prevalence of diabetes throughout Delaware, with the most dramatic increase in Sussex County. In an effort to address this growing health concern, the Healthier Sussex County Task Force has developed the diabetes care pilot program.
